GPH Announces COVID-19 Vaccine Clinics for 16 & 17 Year Olds

[sent by Chardon Local Schools on 4/21/2021 via Blackboard Connect email to all district staff and parents/guardians]

Geauga Public Health has announced upcoming COVID-19 vaccination clinics for 16 and 17 year olds.  Please note that these clinics are organized by Geauga Public Health. Therefore, any communications regarding registration for the clinics, vaccine questions, etc. should be directed to GPH.

As a public entity, Chardon Local Schools has provided permission for the middle school facility to serve as the location for one of the clinics (both dose #1 and dose #2).

The dates and regional hubs for these clinics are as follows:

  • Saturday, April 24, Chagrin Falls High School
(dose #1 with auto-scheduling of dose #2 for three weeks later - same location - Saturday, May 15)

  • Saturday, May 1, at Chardon Middle School
(dose #1 with auto-scheduling of dose #2 for three weeks later - same location - Saturday, May 22)

Registration:

  • Parents/guardians who are interested in having their 16 or 17 year old child vaccinated are responsible for setting up their child's appointment (as vaccination requires adult permission).

  • Appointments for dose #1 for either location can be made by visiting:  https://vaccine.gphohio.org/

  • Both COVID-19 doses at both locations will be Pfizer vaccines. As noted above, the second dose is automatically scheduled for the same time and location three weeks later.

  • Both the Chagrin Falls and Chardon clinics will be available for any 16 or 17 year old in Geauga County (with written parent/guardian consent).

  • Due to limited doses, these two GPH clinics will only be vaccinating individuals who are 16 or 17 years old.
